Becoming a Member of Gateway Clubhouse
How do I become a member?
If you are interested in becoming a member, you should contact Denise Johnson or Lauren Cox at 919-662-0940 or by email to gain more information about the Gateway Clubhouse.
- Complete a Gateway Clubhouse referral form and sign release to communicate with appropriate LME and return to staff at the Gateway Clubhouse for initial screening to determine eligibility.
- Gateway Clubhouse will send initial paperwork to appropriate LME to determine/confirm funding eligibility.
- Formal referral from the LME will be presented to Community Workforce Solutions Admission Committee to formally determine eligibility for services at the Gateway Clubhouse.
- Complete a tour of the Gateway Clubhouse in order to observe the program and allow current members to determine final acceptance into the program.
- Arrange appointment to complete initial paperwork and review policies, procedures, and rules with individual and/or guardian.
- Schedule a start date to begin participation at the Clubhouse.
